13 - SUPERVISED PRACTICE SITES, PRECEPTORS AND LETTER OF AGREEMENT

13.1 - SUPERVISED PRACTICE SITESUpdated: 1/29/2019

Based on the number of supervised practice hours needed, Participants must arrange his/her own Supervised Practice Sites and have Sites approved by ISPP Coordinator no later than 4 weeks before rotation begins. They must also submit a calendar with probable dates that documents when supervised practice hours will be carried out. Participant must finish all supervised practice hours within two (2) years which is 24 months from the date the first supervised practice experience begins. Failure to complete all supervised practice hours within this time will result in additional cost.

Participant must comply with any and all policies required by the Supervised Practice Sites. Those may include but are not limited to HIPAA or other required trainings, background checks, drug screens, health insurance, immunizations, dress codes, orientations, and work schedules.

Participant may be removed from the Supervised Practice Site if the ISPP Coordinator determines in his/her sole judgment that the Participant is not likely to successfully complete the supervised practice hours with additional learning modules and/or supervised practice.

13.2 - PRECEPTORSUpdated: 1/29/2019

Preceptors are responsible for your Supervised Practice Experiences. It is the responsibility of the Participant to arrange his/her supervised practice hours for the clinical, management, and community experience (How to Find Preceptor Form). Participant must provide the ISPP Coordinator with Preceptor information (name, phone number, email address and postal address) for each rotation at least 4 weeks before Supervised Practice Experience begins. Each Preceptor has to complete a Preceptor Qualification Form which will be returned to the ISPP Coordinator.

13.3 - LETTER OF AGREEMENTUpdated: 1/29/2019

Once it is determined that the Preceptor can provide adequate supervision, the ISPP Coordinator will contact the Preceptor to obtain a Letter of Agreement (LOA) with the Supervised Practice Site and the Preceptor, along with the Preceptor’s resume. No rotation can begin until LOA and resume are on file.  Failure to provide required information in a timely manner may delay Participant’s start time and projected ISPP completion date. While at the Supervised Practice Site, Participants should not be compensated for their time and must not be used to replace employees.
